All new care cases are thoroughly assessed by our fully qualified Care Management Team and they ensure the needs and preferences of the individual and family are considered.

The care package is formulated with professionalism and sensitivity and it is tailored to the requirements of the individual. The care can be structured around specific timeframes of activities. Our aim is to enable the individual to retain their independence and dignity, whilst continuously adhering to our Principles of Service.

The Care Management Team also works collaboratively with other health professionals and agencies to ensure a seamless service of care for the individual.

The care is managed and monitored continuously to ensure that the best quality of care is provided by our staff and that concerns raised by the individual or an organisation are responded to immediately and rectified.

The types of services Bridges Healthcare provides include:

24 hour care

This is staffed by either nurses or carers, or a combination of both. This care would be appropriate for continuing care patients who have high dependence needs, individuals receiving palliative care and individuals who have physical and mental long term chronic disabilities.

Community Care Packages

A care package is formulated to facilitate independent living and support daily activities. The services available are: attention to personal care, preparation of meals, shopping and housework duties.

Respite Arrangements

A sitting service is provided during the day or for a longer duration to allow an informal carer to take a break.

Convalescence Care

To enable an individual a period to recover and receive short term rehabilitation.

Night Sitting

Active or sleep over cover is provided.
