Nurse Team Leader / Nurse Specialist (Substance Misuse - Community)

A band 6 Nurse Team Leader / Nurse Specialist to work with service users with complex substance misuse needs in the community, in clinics and hospital settings.

The Role:
Assesses patients, plans and implements individual care programmes and therapeutic interventions in a range of community settings, clinics and hospital.
Provides specialised advice in the community and maintains associated, accurate and timely records.
Manages a caseload and functions as a Care Co-ordinator for service uses with complex drug/alcohol and healthcare needs.
Liaises with other agencies in planning and assessing programmes of care.
Provides individual support for people with substance misuse needs to enable them to live in the community and make their own choices.
Works within an interdisciplinary team to deliver quality care with an emphasis on promoting independence, rehabilitation and harm reduction.
Carries out nursing procedures.
Provides clinical supervision to other staff, students.
You are required to be registered on the appropriate part of the NMC register as required for the assignment: RN (Mental Health ) RN3, RN4 or RN1, RN2
You will need the relevant diploma level specialist training or equivalent as required for the assignment, evidence of continuing professional education, evidence of professional updating.
You need a minimum of two years post registration experience, at least 1 year (within last 6 months) in Substance Misuse Services / in the field of the assignment and crisis management and intervention experience. A mentorship / teaching & assessing qualification and experience in phlebotomy is desirable but not required.
