Nurse Team Manager (Community)

Nurse Team Manager required to work with members of the community.
Ref: CS20/G7
You will work with members of the community: in the home, in clinics and in intermediate care settings, managing a team of community nurse specialists and other staff, covering a geographical area, including recruitment and appraisal.
You will assess patients, plan and implement care in the community, as well as provide skilled, professional assessment of health needs, relevant nursing care and health information to members of the community. Providing advice and maintaining associated records you will carry out nursing procedures and provide clinical leadership to the community nursing team and co-ordinate nursing team workloads.
You will organise community placements for students, provide induction training, and do clinical supervision. Working in partnership with the wider primary care team and social services you will deliver quality care with an emphasis on promoting independence and rehabilitation as well as curative and palliative care.
You will have a minimum of four years post registration experience, at least 2 years experience within the field of assignment, current (within last 6 months) evidence based practice and experience in staff management and supervision.
