Working with patients in the community: in the home, in clinics and in intermediate care settings you will assess patients, plans and implements care in the community, provide advice and maintain associated records,  You will also carry out nursing procedures and may provide clinical supervision to other staff and students.

You will work within an interdisciplinary team to deliver quality care with an emphasis on promoting independence and rehabilitation as well as curative and palliative care, and work within codes of practice and professional guidelines.  Work is managed rather than supervised.
