Nurse Associate Practitioner (Community)

Nurse Associate Practitioner required to work with patients in the community who have differing care needs.
Ref: CS4/G4
Working with patients in the community: in the home, in clinics and in intermediate care settings you will implement planned care for patients in the community and other settings; observe patients' conditions, record clinical observations and report to qualified nurses. You will contribute to the assessment of care needs and the care plan.
You may also carry out some designated nursing procedures ( when trained and competent to do so), supervise clinical support workers in the same area and assist in assessment and attainment of learning and development needs of clinical support workers.
You should have at least 3 years experience in the area of the assignment and be able to deliver skilled, clinical care to patients based on patient need and according to the plan of care.
