Working with patients in the community including older people, people with physical disabilities, sensory impairment, terminal illness, dementia, mental health problems, children and their families you will provides personal care for patients in the community: within the home, in clinics or in intermediate care settings.

You should have proven experience in a hospital / care / nursing environment at band 2 level or equivalent (at least 6 months).
